Commercial Waterway User Sticker Application

Chapter 4, Sections 4.01 and 4.02B of the Fox Waterway User Permit Sticker Regulations state that it is unlawful for any person to engage in Commercial Operations on the Fox Waterway without first having obtained a Commercial Waterway User Sticker. To apply for a Commercial User Sticker, complete and submit this Application with all required attachments through the Agency’s online platform or in person at the Fox Waterway Agency’s office. All sections of this application must be filled out truthfully and in their entirety. If a Section does not apply to you, mark “N/A”. Incomplete forms will not be accepted or processed by the Agency. It is unlawful for any business to provide false or fictitious information on record related to your registered vessels.

Commercial Boat Operations Information

It is unlawful under Illinois law for any person to engage in Commercial Operations without first having obtained any State required license and registration “Commercial Operations,” including commercial boat categories, are defined in the Agency User Permit Sticker Regulations (Ch. 4 and Appx. A). You must submit all required state registrations and licenses with this Application.
